The service relationship between you and Anchored Whole Health, LLC — including consent to services, scope of practice, payment, and cancellation — is governed by the separate Client Services Agreement provided and signed through our Practice Better client portal prior to your first session.

By accessing our website or using our services, you agree to the practices described in this Privacy Policy. If you do not agree, please do not use our website or services.

2. Who We Are

Krista Klosowski, RN, CNS, LDN is the owner and sole practitioner of Anchored Whole Health, LLC, a virtual health and nutrition practice. We provide the following services to clients throughout the United States (and in-person services on the Treasure Coast of Florida):

–     Health Navigation Sessions — educational and advisory sessions to help clients process health concerns, understand their options, and plan next steps

–     Functional Nutrition Coaching — personalized nutrition assessment, lab interpretation, diet and lifestyle planning, and ongoing coaching support

–     Supplement Review Sessions — comprehensive review of current supplements and medications, interaction assessment, and evidence-based recommendations

–     Health & Wellness Q&A Sessions — general health, nutrition, and environmental wellness guidance in an open question-and-answer format

–     Grocery Shopping Tours — virtual grocery store walkthroughs to build label-reading skills and health-supportive shopping habits (available in-person on the Treasure Coast of Florida only)

3. HIPAA & Health Information

As a licensed healthcare provider, we are committed to protecting the privacy and security of your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state laws.

What is Protected Health Information (PHI)?

PHI includes any information that relates to your health condition, the healthcare services you receive from us, or payment for those services, and that can be used to identify you. This includes your name, contact information, health history, diagnoses, lab results, treatment information, and appointment records.

How we use your PHI

The nature of the information we collect varies by service:

–     Health Navigation Sessions: health history, diagnoses, treatment plans, provider information, and health concern documentation

–     Functional Nutrition Coaching: comprehensive health history, bloodwork and lab results, dietary assessment, lifestyle information, supplement and medication lists, and session notes

–     Supplement Review Sessions: current supplement and medication lists including names, doses, and timing; bloodwork or functional lab results uploaded prior to the session

–     Health & Wellness Q&A Sessions: general health and wellness background, questions submitted prior to or during the session, and any health context shared during the appointment

–     Grocery Shopping Tours: basic health goals and dietary needs provided in the pre-tour intake form. Note: grocery tours conducted in person at a retail location will not collect PHI or share PHI at the store itself.

Permitted uses of your PHI

We use your health information only for the following permitted purposes:

–     Treatment: To provide, coordinate, and manage your health, nutrition, and wellness services

–     Healthcare operations: To conduct internal quality reviews and administrative functions necessary to run our practice

We do not accept insurance and do not bill any third-party payers. Your health information is not shared with insurance companies or payers in connection with your care at Anchored Whole Health.

We do not sell, rent, or share your PHI with third parties for marketing purposes.

Notice of Privacy Practices

Prior to receiving services, clients are provided with our Notice of Privacy Practices through our secure HIPAA-compliant client portal. Clients are asked to review and acknowledge receipt of this notice as part of the onboarding process. A copy is available upon request at any time.

Your HIPAA rights

Under HIPAA, you have the right to:

–     Access and obtain a copy of your health information

–     Request corrections to your health records

–     Request restrictions on how we use or disclose your information

–     Receive an accounting of disclosures of your PHI

–     File a complaint if you believe your privacy rights have been violated

Information you provide directly

We collect information you provide when you:

  • Complete an intake form or health history questionnaire prior to any session

  • Submit a pre-tour intake form for a Grocery Shopping Tour

  • Book an appointment through our scheduling platform, Practice Better

  • Submit contact forms or send us an email or message

  • Share health records, lab results, supplement lists, or other documents with us through the Practice Better client portal

  • Communicate with us during or between sessions via our client portal

This information may include your name, date of birth, contact details, health history, current medications, supplements, symptoms, goals, lab results, and payment details. The specific information collected varies by service as described in Section 3 above.

Information collected automatically through the website

When you visit our website, certain information may be collected automatically by our website platform or third-party analytics tools, including:

  • IP address and general location

  • Browser type and device information

  • Pages visited and time spent on the site

This information is used for website analytics and security purposes only and is not linked to your personal health information.

Information collected through Fullscript

The Anchored Whole Health, LLC Fullscript practitioner supplement dispensary is accessible to all website visitors — no appointment or existing client relationship is required. If you choose to create a Fullscript account or make a purchase through our dispensary link, please be aware of the following:

  • Fullscript is an independent third-party platform. Any information you provide when creating a Fullscript account — including your name, email address, shipping address, and payment information — is collected directly by Fullscript and is subject to Fullscript's own privacy policy, available at fullscript.com/privacy

  • Anchored Whole Health, LLC does not collect, receive, store, or have access to your Fullscript account details, payment information, or order history

  • For existing clients, we share only your name and personalized supplement protocol with Fullscript for the sole purpose of delivering your recommendations through the platform — we do not share your full health record or any other personal health information with Fullscript

  • We encourage all visitors to review Fullscript's privacy policy before creating an account or making a purchase

Information you choose not to provide

You are not required to provide any personal information to browse this website. However, certain features — such as booking an appointment or accessing your client portal — require you to provide information in order to use them. Choosing not to provide requested information may limit your ability to use certain services.

5. How We Use Your Information

We use the information we collect to:

–     Provide, personalize, and improve our services across all five service offerings

–     Prepare for your session by reviewing intake forms, uploaded labs, supplement lists, and other documents you share in advance

–     Communicate with you about your appointments, care plan, and follow-up

–     Process self-pay session fees

–     Send appointment reminders and administrative notices

–     Comply with legal and professional obligations

–     Improve our website and service quality

We do not use your health information for advertising, sell it to third parties, or share it for purposes beyond those listed above without your explicit consent.

6. How We Share Your Information

We do not sell your personal information. We do not share your information with insurance companies or third-party payers. We may share your information only in the following limited circumstances:

Service providers and business associates

We work with third-party vendors who help us operate our practice. Our primary platform is Practice Better, our HIPAA-compliant electronic health record (EHR) and client management system. Practice Better is used to manage appointment scheduling, intake forms, client communications, session notes, document storage, and the delivery and signature of your Client Services Agreement and consent forms. Practice Better has signed a HIPAA Business Associate Agreement (BAA) with us and is required to maintain the confidentiality and security of your information in accordance with applicable law.

We also use a HIPAA-compliant video platform for virtual sessions, which has also executed a BAA with us. All other third-party vendors who may access your information in the course of providing services to our practice are similarly required to maintain appropriate confidentiality and security standards.

Supplement dispensary

The Anchored Whole Health, LLC Fullscript practitioner supplement dispensary is accessible to all website visitors, not only current clients. If you choose to create a Fullscript account or make a purchase through our dispensary link, you are doing so directly through Fullscript, a third-party platform. Anchored Whole Health, LLC does not collect, store, or have access to your Fullscript account information, payment details, or order history. All information you provide to Fullscript is subject to Fullscript's own privacy policy, which we encourage you to review before creating an account at fullscript.com/privacy. For existing clients, we share only your name and recommended supplement protocol with Fullscript for the purpose of delivering your personalized supplement recommendations through the platform.

Legal requirements

We may disclose your information when required by law, court order, or governmental authority, or when we believe disclosure is necessary to protect the safety of you or others.

With your consent

We may share your information with your other healthcare providers or designated individuals with your explicit written consent.

7. How We Protect Your Information

We take the security of your health and personal information seriously. We use the following safeguards:

–     All virtual appointments are conducted on a HIPAA-compliant, encrypted video platform

–     Health records and intake forms are stored within Practice Better, our HIPAA-compliant EHR and client management system

–     Electronic communications containing PHI are transmitted through secure, encrypted channels

–     Documents and lab results uploaded by clients are stored securely within the Practice Better client portal

–     Access to your health records is limited to authorized personnel only

–     Grocery Shopping Tours conducted in person at retail locations; no PHI is collected, displayed, or discussed in a public setting during tours

No method of electronic transmission or storage is 100% secure. While we take all reasonable precautions to protect your information, we cannot guarantee absolute security. If a data breach occurs that affects your PHI, we will notify you as required by applicable law.

8. Data Retention

We retain your health records and personal information for the period required by applicable federal and state laws, professional licensing requirements, and our record retention policies. The retention period may vary depending on the state in which services are provided and the nature of the records.

When the applicable retention period has expired, records will be securely destroyed or permanently de-identified in accordance with applicable law.

Website analytics data and other non-health information are generally retained for up to 10 years or until you request deletion, where applicable and permitted by law.

9. Your Rights

Depending on your state of residence, you may have the following rights regarding your personal information:

–     Right to access: Request a copy of the personal or health information we hold about you

–     Right to correction: Request that we correct inaccurate or incomplete information

Our website and client portal may contain links to or integrations with third-party platforms, including Practice Better (EHR and scheduling), Fullscript (supplement dispensary), and our video conferencing platform. We actively direct website visitors to our Fullscript practitioner supplement dispensary. Fullscript is an independent third-party platform — once you click through to Fullscript, you are subject to Fullscript's terms of service and privacy policy. We have no control over and accept no responsibility for Fullscript's data practices, product availability, order fulfillment, shipping, or any disputes arising from purchases made through the platform.

12. Telehealth and Virtual Services

Virtual services

The following services are delivered entirely via telehealth: Health Navigation Sessions, Functional Nutrition Coaching, Supplement Review Sessions, Health and Wellness Q&A Sessions, and Grocery Store Tours/Shopping Support. By using our telehealth services, you acknowledge and consent to:

–     The use of electronic communications and video technology for your appointments

–     The inherent limitations and risks of telehealth services, including potential technical issues

–     The fact that your sessions may be conducted while you are in a different state than where we are licensed

 We take reasonable steps to ensure a private and secure connection for all virtual sessions and recommend that you participate from a private location.

In-person services — Grocery Shopping Tours

Grocery Shopping Tours may be conducted in person at a retail grocery store location on the Treasure Coast of Florida. The following applies to in-person services:

–     No PHI is collected, recorded, or displayed during the in-person tour itself

–     Health goals and dietary information shared via your pre-tour intake form are collected through our secure Practice Better portal prior to the session

–     Tour sessions take place in a public retail environment; please be mindful of what personal health information you choose to discuss in a public setting

–     In-person tours do not involve video recording or audio recording of any kind

13. Changes to This Privacy Policy

We may update this Privacy Policy from time to time to reflect changes in our practices, technology, or legal requirements. When we make material changes, we will update the effective date at the top of this page and, where appropriate, notify you by email.

Your continued use of our website or services after any changes take effect constitutes your acceptance of the updated policy.
